Beach Street Health and Rehabilitation Center - a 99 bed Skilled Nursing Facility located in Daytona Beach, FL is currently seeking a Social Services Director. - We are a growing facility in need of an energetic and talented new professional to join our team.
As the Social Services Director (SSD) within a Skilled Nursing Facility, you will ensure that the Social Services department remains focused on the continuous efforts to develop and improve the overall well-being of residents. - To be most successful in the role of Social Services Director, you should be a goal-oriented TEAM player who takes pride in leading by example. - -
Social Services Director Job Summary & Qualifications:

• 
  
• Collect data used in discharge plans, MDS and Care Plans
  
• Assist new residents in adjusting to the facility.
  
• Coordinate and Lead staff meetings and other family and group meetings.
  
• Required: Minimum 3-5 years' experience in social working in a nursing home.
  
• Ability to communicate at all levels of the organization, and work well within a TEAM environment
  
• Customer service oriented with the ability to work well under pressure.
  
• Ability to work with minimal supervision, take initiative and make independent decisions.
  
• Approachable, flexible, highly adaptable to change -
