*Conviva Senior Care Centers* is seeking a Board Certified/Board Eligible Advanced Practice Provider for an outpatient geriatric primary care position in Port Orange, Florida.*The Conviva Difference:*As a Provider with Conviva Senior Care Centers, you'll have the opportunity take a hands-on approach to addressing patient needs inside the exam room, as well as looking beyond to understand how social determinates of health are impacting health outcomes. We are experts in senior-focused primary care with a multidisciplinary approach.*A day in the life of a Conviva provider:** See 12-15 patients per day. Opportunity to spend more time with patients* Thrive in a value-base care model and improve patient's quality of life* See a variety in your caseload, from preventative care to chronic conditions, with a focus in this role on walk-in and same day, emergent visits* Spend 100% of your time clinically focused on direct patient care, inclusive of patient facing time and general administrative time (charting, meetings, etc.) as it relates to direct patient care*Your Accomplishments are Valued:** Active, unrestricted NP/PA license in the appropriate state* Current certification as an NP or PA* Current DEA license or ability to obtain a DEA license* This role is considered patient facing and is part of Humana/Senior Bridge's Tuberculosis (TB) screening program.
