Job Description:

Provides case management for assigned patients, utilizing clinical expertise, communication, and problem-solving skills to optimize outcomes.
Performs needs assessments, facilitates referrals, and links patients to health, wellness, and post-acute resources across the care continuum.
Promotes interdisciplinary collaboration, care coordination, and discharge planning to ensure appropriate length of stay, resource use, and smooth transitions.
Ensures compliance with regulations, documents accurately, and communicates with team members, including utilization review nurses and social workers.
Facilitates referrals for medications, DME, home health, and hospice; manages barriers and authorization processes.
Assists with transfers, advocates for patients, and participates in performance improvement initiatives.
Uses predictive analytics, manages post-acute care needs, and provides medication support.
Requires RN licensure, a bachelor's degree or equivalent experience, and 3+ years clinical case management experience.
