Highlights Deliver comprehensive, patient-centered care for a
senior-only panel Monday to Friday schedule, 8:00 AM to 5:00 PM
with a dedicated lunch hour Patient volume: 10-15 per day, allowing
for quality, unrushed visits Focus on preventive care, chronic
disease management, and patient education Close collaboration with
supervising physicians and a multidisciplinary care team Practice
medicine in a model designed around outcomes, not volume Practice

Dedicated care coordination, health coaching, and pharmacy support
Streamlined EMR workflows for efficient documentation and reduced

Nurse Practitioner program with active licensure Bilingual

hypertension Skilled in EMR documentation and committed to accuracy
Compassionate, proactive, and patient-centered in approach Strong
