This vital role supports our mission of providing expert, whole-person care by managing patient care plans, ensuring seamless transition between care settings, and maximizing health outcomes. The RN Case Manager will collaborate with multidisciplinary teams to assess patient needs, establish care objectives, and evaluate patient progress. Assess patients regularly to determine their needs and ensure appropriate care is provided promptly. Develop, implement, and evaluate individualized patient care plans in collaboration with other health care professionals. Act as a liaison between patients, families, and healthcare providers to ensure effective communication and understanding of treatment plans. Monitor patient progress and adjust care plans as necessary. Provide education and guidance to patients and their families about health condition management, medication, and prevention strategies. Ensure compliance with healthcare laws, regulations, and standards. Strong interpersonal and communication skills Proficient in case management software and medical records technology Ability to work effectively within an interdisciplinary team Strong problem-solving and organizational skills Medical Insurance Dental Insurance Vision Insurance 401k Current Registered Nurse (RN) license in the state of practice Case Management Certification (e.g., Commission for Case Manager Certification or American Case Management Association Certification) preferred
