Job Description Job Description Now Hiring: Night Case Manager About Five Star Veterans Center Five Star Veterans Center is a residential transitional housing program serving up to 35 displaced and at-risk veterans in Jacksonville, Florida. Through our Passport to Independence Program, we restore hope and independence for veterans through stability and purpose. We provide safe housing, structured programming, and supportive services that help veterans transition to sustainable independent living. We are currently seeking a Night Case Manager to join our team. Position Overview The Night Case Manager provides professional case management services while serving as the primary on-site staff member during overnight hours. This role ensures resident safety, maintains 24/7 supervision in compliance with VA Grant and Per Diem (GPD) requirements, and supports veterans in achieving their individualized service goals. This is a full-time overnight position. Schedule Five (5) 8-hour overnight shifts per week Typical schedule: 9:00 PM – 5:30 AM (subject to operational needs) 40 hours per week Key Responsibilities Case Management Responsibilities Maintain and/or support caseload of veterans Develop and/or update Individual Service Plans Complete case notes and documentation in accordance with VA GPD standards Support housing stabilization and employment readiness Coordinate with internal program staff and community partners Participate in case review meetings Overnight Supervision Responsibilities Remain on-site during assigned shift Monitor facility security and AI-based surveillance system Conduct required nighttime facility checks Respond to resident incidents and concerns Enforce house policies and quiet hours Contact emergency services when necessary Document incidents and required logs Qualifications Required: Associate’s degree in social services, behavioral health, or related field (or equivalent experience) Crisis intervention training De-escalation training CPR/First Aid certification Current NARCAN certification Certified Recovery Support Specialist (CRSS) or equivalent credential Preferred: Experience in residential or transitional housing programs Experience working with veterans, homeless populations, or behavioral health clients Compensation Salary range: $40,000 – $50,000 annually (based on experience) Full-time position (40 hours per week) Professional, mission-driven work environment Work Environment Residential facility setting (20,000 sq. ft. campus) Overnight supervision and monitoring responsibilities Must be able to walk facility grounds and respond to emergencies Why Join Five Star?
